To be eligible for the 2007 CPRHA Sire Stakes, this form must be filled out completely & returned with a check for $100 if postmarked before January 1, 2007, $200 if postmarked January 2 thru June 1, 2007, or $300 if postmarked after June 1, 2007.  Please declare Open or Non-Pro, Futurity/Maturity Entry.
