To be eligible for the 2007 CPRHA Sire Stakes, this form must be filled out completely & returned with a check for $250 if postmarked before January 1, 2007; $350 if postmarked between January 2, 2007 and April 1, 2007, or $500 if postmarked after April 1, 2007.
